About Your Visit

At Pro-Motion physical therapy every patient visit is very comprehensive and thorough. On your initial evaluation we will explain to you how we are going to evaluate the condition you are coming to physical therapy for. We will explain to you in detail your physical therapy diagnosis and how we got to that diagnosis. We will identify what structure or combination of structures is giving you your pain. We will then discuss the treatment options available to treat your condition and why we would choose one over the other for your specific condition. At that time we will make clear your expected outcome. If the condition you were referred to physical therapy is not something we can help you with, we will also let you know at the time of your initial evaluation. We believe an educated patient achieves better results.

Please bring to your appointment:

Physician referral/prescription.
Current photo ID.
Your insurance card/information.
Please wear appropriate clothing so we can access the area you need us to treat.
Any MRI/CT scan or x-rays you have in your possession related to your condition.
A list of all your medications. Only clean gym shoes are allowed in the physical therapy clinic.

You may print out and fill out the three patient registration forms prior to your first appointment, or fill them out in the clinic prior to your appointment with the Physical Therapist:

If you prefer to fill the forms out in the clinic, please allow 10-15 minutes for patient registration prior to you appointment time with the Physical Therapist. We offer early morning, evening and weekend hours to fit the busiest schedules. Our office is handicapped-accessible and has easy parking. Pro-Motion Physical Therapy is a 3600 square foot state-of-the-art rehabilitation facility. Our patients appreciate our on-time appointments.

There’s one simple reason we’re willing to take you for a ride: We don’t want anything to keep you from the one-on-one physical therapy you need to make a 100% recovery.

So if your injury keeps you from driving, or for any other reason, you’re unable to arrange for transportation to and from Pro-Motion Physical Therapy’s location, we’ll arrange for a driver to pick you up and bring you home.
